BY PURCHASING TICKET(S) YOU AGREE TO CENTENNIAL COLLEGE ALUMNI ASSOCIATION INC. WAIVER AND RELEASE In consideration of being allowed to participate in any way in Centennial College Alumni Association, related events and activities, the undersigned acknowledge, appreciates, and willingly agrees that: I have read this waiver and release in its entirety prior to signing below, and understand that by purchasing below, I am waiving and releasing any and all rights which I, my dependents, heirs, successors, assigns, executors or administrators might have, and I understand that I am releasing the Centennial College Alumni Association Inc., and all of its Officers, Directors, Servants and Agents (collectively the “Releases”) from any liability whatsoever arising out of or in connection with my participation in the CCAA Goes to (Canadian Open)National Bank Open - Toronto (the “trip”), including any liability due to the negligence of the Releases’ or of a breach of contract by the Releases in the course of my participation or involvement in the trip. In consideration of being permitted to participate in the trip, I, on my own behalf and on behalf of my dependents, heirs, successors, assigns, executors, and administrators, hereby release and forever discharge the Releases and waive any rights which I might have against the Releases with respect to any claims and liabilities whatsoever, without limitation, in connection with all activities associated with my participation in the trip including any loss, damage, injury or expense, whether permanent or temporary, suffered by me. I further confirm that I am over the age of 19, and I do have government-issued photo ID to prove my age. PHOTO CONSENT I do hereby give Centennial College Alumni Association, its assigns, licensees, and legal representatives the irrevocable right to use my name, picture, portrait, or photograph in all forms and media and in all manners, including composite, for advertising, for publication or any other lawful purposes, and I waive any right to inspect or approve the finished product, including written copy, which may be created in connection therewith.
